Appointments

Our staff takes appointments in advance and makes every effort to see patients as quickly and efficiently as possible. Please make appointments between 9:00 AM- 5:00 PM. Appointments are given on a first come, first serve basis. In case of an urgent need for an appointment notify the receptionist, these appointments can sometimes be accommodated. We do not accept walk-in patients.

Please contact our office within 24 hours when canceling office visits or consults and at least 48 hours notice if you will be canceling a procedure. We charge a $75 fee for cancellations that are not made within 48 hours period. The second cancellation of a procedure will be a charge of $125.

Prescriptions

If you are an established patient, you can request prescription refills through the patient portal.  If you have not registered for the patient portal available to you, please call our office and you will be given access to your records.

Results

Dr. Llaneza or a staff member calls results to the patient. Results are called within 5 working days of most procedures. The staff or Dr. Llaneza will usually call with normal or abnormal results. If you have not received a call within a week, please contact our office.

Insurance and Billing

We are a fee-for-service practice; therefore, payment is due at each appointment.

Our physician participates in most private plans and some managed healthcare plans as a gastroenterology specialist. It is the patient’s responsibility to inform the receptionist at the time of making the appointment of his/her insurance plan. It is of utmost importance that accurate insurance information is provided at the time of the appointment. We reserve the right to verify eligibility. Please provide your insurance card and government issued picture identification for verification purposes.   Co-payments and deductible, if applicable, will be charged at the time of service.

We are a Medicare participant provider and accept assignment. We file insurance claims as a courtesy, and we will file a secondary insurance if applicable. The patient is responsible for the yearly deductible and the 20% of allowable fees unless a secondary insurance covers this. We are not providers of Florida Medicaid Program.

Some procedures and diagnoses are not covered by some insurance plans. If this should occur, the patient will be informed and asked to sign a waiver stating that he/she understands and will be responsible for any amount not covered by their insurance.

HMO/Managed Care plans may require an authorization to see a specialist. It is the patient’s responsibility to acquire the initial authorization for the first visit and subsequent follow visits. We request that the patient brings it at the time of the visit or make arrangements with the primary care physician to have it faxed to our office. Our scheduling staff member will handle all authorizations for procedures.

Payment Options

We accept cash, check, Visa/MasterCard and American Express.

Rights and Responsibilities

1. Privacy/ Confidentiality:
Dr. Llaneza and the staff will protect your privacy and the confidentiality according to HIPPA and any other
terms the patient wishes.  Patient’s information will not be released unless authorized to do so.
2. Respect:
Dr. Llaneza and the staff will treat each patient with the highest level of respect. Likewise, our patients are
expected to treat the staff and Dr. Llaneza with the equivalent level of respect.
3. Provide all accurate information:
It is the patient’s responsibility to provide all current and accurate information for all questions including
medical history.
4. Finance responsibility:
Patient assumes all financial responsibility of paying for services. The patient must know their insurance
policy and if there are any unique exceptions this should be informed to the front desk before the
appointment.
